I understand that Envision follows an employment-at-will policy, in that I or Envision may terminate my employment at any time, or for any reason consistent with the applicable state or federal laws.   I understand that this application is not a contract of employment.  I understand that to be employed I must be lawfully authorized to work in the United States and I must show Envision documents that will prove this.

I understand that Envision will thoroughly investigate my work and personal history and verify all data given on this application, on related papers , and in interviews.  I authorize all individuals, schools, and firms names herein and on my resume, including my current employer, to provide any information requested about me.  I release them from all liability for damage in providing this information.

This release and authorization acknowledges that Envision may now, or at any time while I am employed, conduct a verification of my education, employment history, credit history, and/or motor vehicle records.  In addition, Envision may contact personal references, require that I provide a urine specimen to be tested for the presence of drugs or alcohol, and receive any criminal history record information pertaining to me which may be in the files of any Federal, State or Local criminal justice agency in any state, and/or other information as deemed necessary to fulfill the job requirements.  Also, if an offer of employment has been made,  I authorize the review of my worker's compensation claim history.
